Job summary
We are seeking to recruit an 8 month Fixed Term/Secondment band 6 nurse RMN/RGN/RNLD with dementia nursing experience, a passionate, knowledgeable and dynamic nurse to join our well-established and innovative North of West Sussex Admiral Nurse service based at New Park House in Horsham. As an Admiral Nurse, your goal is to offer specialist nursing assessments, evidence-based person centred interventions and support for family carers of people with dementia.
You will be joining a small nurse led team, providing high-quality care within the dementia care pathway of the trust, complementing and supporting other services such as DOPMHS. You will be employed by Sussex Partnership NHS Foundation Trust and work in partnership with Dementia UK. You will also be responsible for the provision of consultancy to professional colleagues. The role includes use of the Admiral Nurse Competency Framework to support the Development of admiral nurse practice. The admiral nurse is expected to provide face to face assessments for the majority of carers where possible, including attention to risk management and care planning, and working with families and the wider systems that support people living with Dementia.
This post offers an incentive or relocation package up to the maximum of £4000 (pro rata applies and subject to terms and conditions).
Main duties of the job
As an Admiral nurse you are expected to meet the multiple and often complex needs of carers, through on-going assessments of health and well-being, including risk assessments, developing, implementing and evaluating care plans. You will have the flexibility to manage your workload in the community, however you will be expected to spend some time working from the central team base at New Park House.
The role will include participation in and making effective use of and provide clinical supervision in negotiated circumstances. You will also be expected to support students on placements and New staff on induction within DOPMHS services.
Some duties will include arranging and participation in events for family carers.
The service operates Monday to Friday, from 9am to 5pm. Applicants need to be mobile, with the facility to move across North West Sussex.
