Job summary

Are you a registered health or social care professional with a passion for making a difference to people's lives? We are looking for an excited and committed individual with expertise in supporting autistic people and people with a learning disability to join our team as a case manager.

The post aims to ensure the safety and wellbeing of people from Kent, Surrey and Sussex using secure inpatient services both within our geography and further afield, ensuring that treatment and care is therapeutic, purposeful and delivered as close to home as possible.

The post holder will work with hospitals and wider community services to establish clear care pathways which enable safe and timely discharge to the community and achieve a reduction in length of stay.

Main duties of the job

The post holder will be responsible for ensuring safe and effective care pathways are in place for all adults who are autistic and/or have a learning disability within the Kent, Surrey and Sussex secure care network of mainstream and specialist services, within both the NHS and independent sector.

The post holder will be based within the Kent, Surrey and Sussex region - flexibility of working arrangements will be considered on an individual basis. The post involves frequent travel both regionally and nationally.

The post holder will be responsible for working with patients, their families/carers or representatives, ICS partnerships, NHSE regional and national colleagues, NHS and independent contracted providers and other local and regional partners to ensure cohesive care pathways for our clients.
