An opportunity has arisen for a Clinical Nurse Specialist to join the Changing Futures Sussex Programme: one of 15 national Programmes bringing together public and community sector partners to improve the way local systems and services work for adults experiencing multiple complex needs (MCN). The Programme aims to create an environment where individuals experiencing MCN, who repeatedly fall through the net of service provision, receive flexible, trauma informed, person-centred support.
As the Clinical Nurse Specialist, you will use your clinical expertise in assessing and managing clients, working collaboratively in a newly developed multi-disciplinary team alongside colleagues in homelessness prevention, mental health, and social work, as well as with third sector providers.
We're looking for who people will adopt a curious, creative and collaborate approach to service provision. The team is part of a wider Changing Futures Sussex programme, including systems change priorities which aims to identify blocks and barriers in service provision, recognising best practice and flexible working to overcome the challenges facing people with MCN. There is a strong emphasis on learning and practice development, and informing future commissioning across health, housing, and social care.
Main duties of the job
The Clinical Nurse Specialist role will includein-depth mental health assessments,and risk assessments. It will also involve:
- Carrying a small caseload (maximum 10 clients) working collaboratively with other members of the multi-disciplinary team.
- Working closely with colleagues in ATS, and wider Trust colleagues.
- Working with carers, families, and partner agencies.
- Offering expertise on mental health, providing specialist nursing assessments, screening, as well as offering advice to the multi-disciplinary team where required.
- Assessing and managing ongoing risks as identified during the assessment, ensuring that the collaboration is person-centred, trauma informed with the voice of the service user and carers paramount.
- Regularly reviewing risk factors and making changes to the management of risk as necessary.
- Working autonomously as a nurse with support from the wider multi-disciplinary team.
- Supervision of junior staff members (subject to recruitment).
