Job summary

As we work towards our goal of providing an integrated service across primary and secondary care, our community mental health services are being transformed to recognise the changes in how people want to access them. As part of this Sussex Partnership NHS Foundation Trust (SPFT) are working with multiple stakeholders across Sussex to develop the Emotional Wellbeing Service (EWS).

We are seeking an enthusiastic individual who is looking for a new and hugely exciting opportunity to join our Emotional Wellbeing Service in East Sussex as a Team Lead. Employed by Sussex Partnership, you will be experienced in providing effective clinical leadership, working with multiple stakeholders, across all 12 East Sussex Primary Care Networks (PCNs) and you will be required to provide clinical oversight to team caseloads. You will be a qualified Nurse or Allied Health Professional, with the relevant professional registration (NMC, HCPC or SWE).

You will be leading a team of Mental Health Practitioners, Senior Access Facilitators and working alongside a team of Voluntary & Community Sector colleagues that are working across East Sussex PCNs within GP surgeries.

Main duties of the job

A good team leader is someone who is enthusiastic, clinically experienced and motivated to promote the best possible outcomes for our patients. You will be an excellent communicator and will be able to show initiative in the compassionate leadership of a team which is innovative, creative and continually developing.

Along with the generic responsibilities you would expect such as supervising the team, meeting quality targets and working within a specified budget, you will need to be able to build positive, trusting relationships with local Voluntary Sector and Primary Care Network partners andother colleagues both internal and external to SPFT. In addition, you will have aninstrumental role in the further development of our service as part of the community mental health transformation in East Sussex.

The Emotional Wellbeing Service (EWS) offers a practice-based form of mental health support for people with more complex mental health issues and frequent attenders at GP surgeries with longstanding issues. Patients that would not usually be able to access Improving Access to Psychological Therapies (IAPT) services, or meet the criteria for referral to specialist mental health services, can now access EWS.

Forming part of the EWS MDT will be the PHC-SMI Access Facilitator staff who will be proactively working with East Sussex based PCNs/Practices to identify individuals on their Seriously Mentally Ill (SMI) Registers who would benefit from support to engage with the nationally mandated comprehensive Physical Health Check (PHC-SMI).

REQUIREMENTS

In order to be a successful Team Lead in our team you will be:

  • A qualified Nurse or Allied Health Professional, with the relevant professional registration (NMC, HCPC or SWE)
  • Experience in providing clinical leadership
  • Motivated by developing a newly created role, within a new service provision
  • Experienced working within a mental health setting

  • Experienced working in a Primary Care setting
