The Changing Futures multiple complex needs (MCN) service is at the forefront of innovative, person-centred, multi-disciplinary working, striving to provide a pioneering multi-disciplinary assessment and support service.
As the Clinical Nurse Specialist, you will be working in a newly developed multi-disciplinary team alongside colleagues in homelessness prevention, mental health and social work, as well as with third sector providers, building a broad range of experience. Then successful applicant will use their clinical expertise in assessing and managing clients with their colleagues in the team. They will also help identify gaps in services for a cohort of people who's mental health needs are not met.
The team works holistically with people in East Sussex who have multiple complex needs including mental ill health, domestic abuse, homelessness, contact with the criminal justice system and substance misuse.
Main duties of the job
Changing Futures Programme are keen to welcome a nurse looking for band 7 experience and wanting to improve access to a disadvantaged cohort. This role will also emphasis on learning and practice development, and the service supports ongoing work around system change; needs mapping for the multiple complex needs cohort, and informing future commissioning across health, housing and social care.
Clinically the role will include in-depth mental health assessments, risk assessments.
Carry a small caseload (with colleagues from other services).
Work closely with colleagues in ATS, and wider Trust colleagues
Working closely with carers, families and partner agencies
To offer expertise on mental health, providing specialist nursing assessments and screening as well as advice where required.
To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.
