Job summary

An opportunity has arisen for the Crisis Resolution Home Treatment team to offer a Band 5 post to an experienced Clinician, either from a Mental Health Nursing, Occupational Therapy or Social Work background.

We are looking for a "can help" person to help us develop a flexible response to individuals who are either presenting urgent mental health needs or at risk of their mental health deteriorating to the point of crisis, by offering specialist intervention.

We are looking to recruit a Practitioner who can demonstrate competence in risk assessment of individuals presenting in crisis toward formulating clear and safe plans that support a person's presenting needs to be managed in the least restrictive environment, often by offering home treatment. The successful post holder will become part of a supportive, energetic and creative team.

Main duties of the job

Offering a bespoke service to address the needs of those individuals in crisis by formulating care to manage risk in a positive fashion

This role also works closely with and supports the work of other Urgent Care Teams based in same department (Mental Health Liaison and Rapid Response team) to provide an alternative to Emergency Department attendance and hospital admission.

  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development

Knowledge/Experience

  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals

  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
