Job summary

The Forensic Outreach Liaison Service (FOLS) is a community-based service in Sussex providing holistic support to individuals with mental health issues that relates to their offending/risk behaviour. The main focus of this exciting role will be to provide support in the community to individuals with mental health and offending behaviour.

We are looking for an individual who is enthusiastic, able to demonstrate a strong sense of initiative, based in Sussex, adapting to any given situation, who works well as part of a team. You will carry out specialist assessments and devise robust risk management plans, whilst implementing bespoke interventions in your role. The role encompasses both working as an experienced clinician with service users but also by having skills in leadership, supervision and service development and improvement.

We offer a smaller case load, training opportunities whilst you are working in a supportive environment within a specialised team.

We can offer opportunities for continued professional development, regular supervision and support. The FOLS also has regular CPD days which are bespoke to the team.

We will ensure you have a suitable induction period, and training will be provided according to your needs. We also support the option to lease a trust car in our service.

Main duties of the job

You will be employed as either Forensic Community Psychiatric Nurse -/ Occupational Therapist / Social Worker, substantive contract within the FOLS team, based in Sussex. The base is flexible.

You will be working with patients with offending behaviours, this will include those on restriction orders under the Ministry of Justice.

You will provide support with transition from secure hospital into the community.

You will be a Lead Practitioner holding a case load, and providing a specialised range of clinical interventions.

You will monitor, evaluate and modify treatment in order to measure progress and ensureeffectiveness of interventions.

You will undertake clinical assessment, formulation, intervention, hold CPA reviews.

To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and careers.

To regularly review risk and feedback to the MDT

To offer specialist nursing assessment and advice where required.

You will adhere to the team and Trust's lone-working policies at all times.
