Job summary

A post has arisen for a Community Dementia Liaison Nurse to work for the Proactive Care South service based at Swandean, mostly working in the Lancing and Shoreham areas.

The post holder will part of the Proactive+(PAC) team working within a Primary Care Network (PCN), liaising closely with GP's, community nurses, physio's, OT's and other professionals. You will provide advice to, and, in consultation with other members of the PAC team, help to assess, review and provide care plans for patients with cognitive impairment or a diagnosis of Dementia in a community setting to help reduce the need for referral to secondary care mental health services.

Main duties of the job

SPFT Role within Proactive Care +

SPFT provides mental health nurses who work with people who have a diagnosis, or suspected diagnosis, of dementia within the integrated Proactive Care + Service. All referrals come through the Proactive Care + co-ordinators.

The role of SPFT nurses in Proactive Care + includes:

  • Provide advice, consultation, support and shared care for frail adults who have dementia.
  • Provide assessment/review which may reduce the need for a secondary care referral.
  • Support the diagnosis of moderate to severe dementia for those people who live in care homes, using a range of assessment tools including the DiADem tool.
  • Signpost carers to an assessment of their own needs.

About us

Community Dementia Liaison are based within Coastal Primary Care Networks (PCN) across the coastal CCG. We work in a multidisciplinary way alongside GP's, Community nurses, OT's, Physiotherapist and Community pharmacists, Our client group will have a working diagnosis of dementia and will be on the frailty register.
