Community Mental Health Practitioner (Triage)

Job summary

This is a new role in the newly extended Triage & Assessment Team. This role does not involve case load work (no Lead Practitioner responsibility). The team manages all incoming referrals from GPs, mental health services, Adult Social Care, third sector organisations and takes part in the triage hub with these services to ensure patients get treatment from the service that best meets their needs.

Following initial referral, triage staff conduct risk screening over the phone with patients and make decisions about what happens next. In addition, the newly extended team will now be conducting all assessments (except those requiring medic/OT/Psychology).

Hours are flexible up to 37.5 per week with the opportunity to work up to 50% of hours at home. Parking costs are paid.

Covid-19 full vaccination may be a condition of employment for this role, unless an exemption applies. The successful applicant may have contact with vulnerable service users and we will therefore seek proof of being fully vaccinated during the recruitment process.

Main duties of the job

To be a successful in this post you will be:

  • A Registered Mental Health Nurse/OT/Social Worker
  • Have good assessment and decision-making skills
  • Excellent at forming relationships with our partners
  • Keen to embrace new ways of working and enthusiastic about improving clinical practice

It would also be advantageous if you have:

  • Experience working with adults with severe mental illness
  • Experience working in a community team

Essential

  • Registration appropriate to the job role
  • Completion of Mentorship Course/ ENB equivalent
  • Evidence of continuous professional development

Knowledge/ Experience

Essential

  • Significant post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervision staff
  • Experience of providing a range of clinical interventions to people in the designated care group
  • Experience of conducting clinical assessments including risk assessments
  • Experience of working consultatively with professionals
