Job summary
We are seeking to recruit a passionate, dynamic, knowledgeable and dedicated Community Mental Health Nurse to join our Specialist Older Adult Mental Health team (SOAMH).
The team is multi disciplinary covering High Weald Lewes and Havens (HWLH). We have bases in Uckfield and Newhaven the successful candidate will be based at Millwood, Uckfield. We aim to support people in care homes as well as their own homes to maximise their independence and well being.
You will be joining a friendly, supportive and knowledgeable team who are dedicated to delivering high levels of care to older adults suffering with functional mental illness or have a diagnosis of dementia.
This is a Monday to Friday role 9am - 5pm however flexible working requests can be considered.

Main duties of the job
As a Mental Health Nurse within SOAMHs you will operate as an autonomous practitioner, undertaking comprehensive assessment and formulation, including risk assessment that leads to an agreed plan of care for clients with a diagnosis of dementia and older people with complexed mental health needs.
You will need the ability to work autonomously whilst still contributing to the team as a whole.
You will be responsible for visiting clients in their own homes as well as care homes and hold a full driving license which is essential to this role.
You will have excellent communication skills and work closely with colleagues in social care, primary care and the voluntary sector You will be responsible for covering duty on a regular basis.
You will be provided with regular supervision and have the opportunity to access further CPD.
