The Brighton and Hove Wellbeing Service is recruiting to a number of Band 6 Nursing, OT, and Social Work roles in its new Primary Care Network Mental Health Team (PCN MHT) and in its established Primary Care Mental Health Practitioner team (PCMHP). Both teams work with people in primary care to provide fast access to mental health expertise at the right level to prevent further deterioration or escalation in need. The people we work with are usually unable to access IAPT services, but may not meet the thresholds for secondary care services.
The PCMHP team are based in Sussex Partnership and GP sites across Brighton and Hove. The team has a strong, well-established group of practitioners and seniors delivering short term one-to-one interventions and evidence-based groupwork alongside Brighton and Hove's Wellbeing College.The PCMHP team works closely with IAPT services, with many people using PCMHP to develop their readiness to access IAPT.
PCN MHT roles are embedded in GP Primary Care Networks. This exciting and evolving service integrates closely with small groups of local GP surgeries, delivering flexible care alongside voluntary sector partners (Southdown), whilst maintaining high standards of support and supervision. The team provides accessible, high quality clinical consultation, assessment, and one-to-one intervention right on the GP's doorstep.
Main duties of the job
Both services will need you to use your excellent assessment and triage skills. You will encounter people experiencing a range of mental health needs, and as a band 6 practitioner you will be sufficiently skilled and experienced to work with them to find the right pathway to support.
In both roles you will communicate closely with GPs, secondary care services (ATS, crisis teams, etc), carers, and voluntary sector networks to make sure people's needs are understood at the earliest point, providing a bridge between primary and secondary care when necessary.
Within the PCMHP team, you will undertake brief interventions and facilitate a range of evidence-based groupwork, negotiating onward care with IAPT, ATS, and other Trust and voluntary sector colleagues as necessary.
The PCN MHT is a developing service presently focussing on flexible one-to-one work delivered in partnership with GPs and the voluntary sector. This exciting team offers opportunity and challenge in equal measure to experienced community practitioners wanting to be at the forefront of the transformation of Brighton and Hove's mental health services.
Your interventions will be vital to preventing deterioration in previously stable patients, supporting people to transition to the right services, and supporting GPs and voluntary sector colleagues to provide safe, evidence-based mental health support. Both teams support remote working options within the needs of the service, and of service users.

These roles will be important to the transformation of community mental health service in Brighton and Hove, and will help to ensure that gaps in service between GPs, the voluntary sector, and secondary tier services can be bridged. We are therefore looking for practitioners who can build partnerships, and can work flexibly, adaptively, and creatively in growing and developing these locally focussed teams.
