We are seeking a Newly qualified or current Band 5 Learning Disability Nurse who is looking to grow and development to a Band 6 nurse role within our team. With support from our nursing team and completing our preceptorship program we will support you to gain the necessary skills and experiencedto become a Band 6 nurse within the team.
If this sounds like the perfect development opportunity for you join our Integrated Community Learning Disability Team in our West Sussex Northern region. As an LD Nurse in our team you will be a key member of our multi-professional health team, working in close partnership with Social Services colleagues to support people who have a learning disability, but who also have complex physical and mental health needs including behaviours that challenge.
Our community learning disability team works across West Sussex to provide specialist community support for adults with a learning disability, their families and carers. Our teams include specialist nurses, occupational therapists, clinical psychologists, psychiatrists and more.
Main duties of the job
In this role you will be providing high-quality assessment, support and advice to people with learning disabilities and their carers'. You will provide and/or arrange access to specialist focused health interventions and monitoring as identified in health care and treatment plans, for example; related to complex physical health, epilepsy, challenging behaviour and mental health.
You will manage your own diary and own case load, working across our patch to support our service users in the most efficient and effective way possible. An ability to be able to travel around West Sussex and sometimes wider areas is essential, as you will need to undertake home visits and attend meetings.
As part of this role you will need to be willing to complete the preceptorship program to develop to a band 6 nurse, you will have ample support from our multi-disciplinary team, senior nursing team and our wider Learning Disability Nursing Service across the Trust. Peer support and CPD are at the forefront of how we operate, with your developmental goals being highly valued and encouraged.
