Ward - Community Link Mental Health Practitioner

Job summary

We are seeking a qualified RMN/ Social Worker or Occupational Therapist to work across community mental health and acute wards to ensure safe transitions between ward and community services. Applicant to be based with ATS Hastings at Cavendish House.

We welcome applications from people with ward experience and Band 5 staff.

This post offers a Golden Hello, or a Relocation package up to a maximum of £2000. (pro rata applies and subject to terms and conditions. Please see attached Protocol document).

Covid-19 full vaccination may be a condition of employment for this role, unless an exemption applies. The successful applicant may have contact with vulnerable service users and we will therefore seek proof of being fully vaccinated during the recruitment process.

Main duties of the job

The role will comprise:

  • Regular attendance on wards
  • Supporting discharge planning
  • Maintain awareness of progress of admissions
  • Completion of CPA3
  • Ensuring referrals to ATS are appropriate and signpost elsewhere if not.
  • Attending ATS MDT meetings to share information

You will need to be highly organised and an excellent communicator.

Essential

  • Registered Practitioner with current registration appropriate to the role
  • Evidence of continuous professional development

Desirable

  • Completion of mentorship course/ ENIB equivalent

Knowledge

Essential

  • Significant post qualifying experience (minimum two years) in the relevant care group
  • Experience of working alongside people with psychosis and complex health and social care problems in the designated care group
  • Experience of working consultatively with professionals
  • Experience of partnership working within and across various statutory and non-statutory teams and agencies

Desirable

  • Supervision of staff
