We are looking for a Pathways practitioner to join the hospital discharge team based at Mill View Hospital/East Brighton Community Mental Health Centre, Brighton General Hospital. The successful candidate will be in unique position of having strong links to East Assessment and Treatment Service and well as Mill View Hospital.
The pathways worker will help support, formulate and execute discharge plans for service users who fall under the care of the East Assessment and Treatment Service when they are admitted for an in-patient episode.
This post offers a Golden Hello, or a Relocation package up to a maximum of £2000. (pro rata applies and subject to terms and conditions. Please see attached Protocol document).
Main duties of the job
The postholder will work collaboratively with both the acute and community clinical teams to facilitate effective discharge planning, supporting communication and planning about a patient's discharge pathway.
