Job summary

The Dementia Assessment Service South team incorporates the coastal area covering Littlehampton to Southwick and north to Henfield, Steyning, Storrington and Pulborough. The team undertakes a person-centred holistic assessment of the persons needs and in partnership with the person's family/Care Home staff develop a treatment/ care plan to maximise the person's independence. The team also aims to support the Primary Care Networks in providing liaison, advice and the provision of education.

The multi professional team includes Administration, Occupational Therapy, Nursing, Psychology and Medical staff who work in partnership with staff from the Alzheimer's Society.

We are looking for a well-motivated person with a passion for working with people and their Carers within this exciting specialism. The majority of clinical staff have been with the team for many years and vacancies do not come around very often. Main duties of the job

The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:

  • To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk. To develop alongside the service user their care plan.
  • To offer specialist nursing assessment and advice where required.
  • To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.

This is an exciting role, that welcomes registered mental health nurses with relevant professional registration who possess strong mental health assessment and therapeutic skills. Previous experience of working in older persons mental health services is desirable although not essential.
