Job summary

We have an exciting opportunity to offer the right person a post in our new and developing triage pathway for people living with dementia and older people who have mental health needs. This is a new initiative which will improve access to our services and provide a bespoke model for the triage of referrals to our community services across West Sussex. Given the current situation we have had to think very differently about access to services and we are pleased to offer be able to offer this as office based with the opportunity for some remote working.

This post offers a Golden Hello, or a Relocation package up to a maximum of £2000. (pro rata applies and subject to terms and conditions. Please see attached Protocol document).

Main duties of the job

This is a clinical role that will offer you the opportunity to provide early engagement with people referred to the service and assist with the effective and timely triage of referrals assessing the patient's needs, determining the priority of care and processing the referrals accordingly. You will work alongside other clinicians and administration staff in the team and will demonstrate excellent communication skills, understanding and kindness to people who will, very often be having their first contact with our services.

You will benefit from previous experience in the community setting; however training can be provided to assist with knowledge and competencies. You would be supported with a tailored development plan, and regular clinical supervision and support from senior colleagues.

This post is for 37.5 hours per week.

This community nurse role will provide a triage and information service to patients referred to SOAMHS in West Sussex. The post holder will be a part of the team that will screen and determine the clinical pathway for all patients referred to the service. The majority of referrals will be made via the electronic referral system.The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, They will demonstrate an understanding of clinical interventions relevant the care group to include education, signposting, pathway planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess need, complex risk, subsequently planning and implementing interventions that will support care delivery. They will provide specialist nursing advice. They will provide clinical supervision to staff and students.

  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development

Knowledge/Experience

Essential

  • Significant post qualifying experience in the relevant care group Dementia Experience
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems including specialist assessments
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of clinical governance
  • Previous experience of working in the community
