People experiencing multiple disadvantage are among the most vulnerable in our communities, and often experience entrenched disadvantage, trauma and health inequalities while experiencing barriers in accessing the support they need as public services struggle to respond.
An excellent opportunity has arisen for a Community Mental Health Nurse, Social worker or Occupational Therapist to join the Rough Sleeping Initiative through Changing Future funding, to assess and support clients who struggle to engage with services the conventional way. You will work alongside the Rough Sleeping Initiative multi-disciplinary team with representatives across variety of statutory services including SPFT, ESH, ASC, CGL, local housing authorities and Probation.
The Changing Futures funding will be utilised to build capacity within the existing East Sussex RSI MDT to enable an improved outreach and engagement response for individuals experiencing multiple disadvantage, (in particular to enable the team to respond to those experiencing multiple disadvantage
Main duties of the job
- Triage mental health cases
- Assessment including outreach
- Risk Assessment (ongoing), reviewing and management.
- Liaison with GP, and other professionals, with recommendations re: treatment and meeting their needs.
- Joint working and representation at meetings including co-existing conditions (dual diagnosis) forum or other meetings where clients with multiple complex issues are being discussed
- Appropriate referrals to services within the SPFT when deemed right timing. Also referrals to other more suitable services outside of SPFT.
- Carry a small caseload for those with significant mental health needs, but not ready to engage with mental health services
- Screening for some specific areas including learning disability, PTSD, Neurological (including acute brain injury), as point of contact,
- Embed trauma informed approaches into working practice, educating others as part of the Changing Future philosophy.
