Job summary

The East Sussex Community Team have an exciting new role, inviting applicants from clinicians who have a special interest in neuro-diversity to support people open to community services that also have neuro diverse needs. This new neuro-diversity role will focus on supporting the community teams by providing co working, consultation and expertise to ATS across East Sussex Assessment and Treatment Services (ATS). The post holder will be based with and managed within the ATS's, principally aligned with Transforming Care Assessment Team (TCAT) staff. TCAT are a small assessment and consultation tertiary service that supports Sussex Partnership clinicians to assist with admission avoidance, and support with facilitating discharges for autistic people that are on the CCG Learning Disability and Autism Programme (LDA) Register. The role will more broadly enhance the community team approach and effectiveness with this cohort of people open to community teams.

Within East Sussex the post covers the areas of Eastbourne, Seaford, Hailsham, Hastings and Rother & High Weald Lewes and Haven ATS.

Main duties of the job

You will provide expertise to adults aged 18 and over with a functional mental health problem, and also have neuro diverse needs ensuring individuals are treated in the least restrictive approach which is consistent with their clinical and safety needs.

You will work alongside multi-disciplinary teams across three ATS to provide care in the community.

The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users by undertaking the following duties:

To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk. To develop alongside the service user their care plan.

To offer specialist clinical assessment and advice where required.

To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. To regularly review risk factors and make changes to the management of them as necessary.

To participate in the planning, development and evaluation of profession specific and local multi-professional services.

To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.

To provide supervision of junior staff and trainees where appropriate.

To act within the relevant professional code of conduct at all times.
