Job summary

An exciting opportunity has arisen to join Lancashire and South Cumbria Foundation Trust as a Band 6 Physiotherapist within the Community Rehabilitation Team and Falls Services.

We are a team of Physiotherapists and Occupational Therapists working closely with the multidisciplinary team to provide rehabilitation to prevent further deterioration in patients' physical functioning and wellbeing and reducing risks of falls. We cover the areas of Chorley, Preston and South Ribble.

The post offers excellent personal and professional development within a well-established and supportive environment.

There are Band 7 Clinical Specialist Physiotherapists who will provide supervision and ensure personal development plans are implemented.

The Community Rehabilitation Team and Falls Service includes;o The Falls Prevention Service which is a multidisciplinary team providing community based assessments for service users who are at risk of falling. Therapists take a holistic approach to treatment following completion of a comprehensive geriatric assessment and provide rehabilitation either at home or in one of our group based exercise classes.

o The Intermediate Care Service works in partnership with Lancashire County Council to ensure patients are safely discharged from hospital. This specialist inpatient team rehabilitates patients back to their base line, before working closely with the individual and their families to facilitate a timely return home.

- Provide a high standard of specialist physiotherapy input to the designated therapy team.

- To assess and treat patients within a ward or community setting as an autonomous practitioner

- Physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions, to provide a diagnosis and develop and deliver an individualised treatment programme.

- Hold responsibility for own complex caseload and be responsible for a defined area of the service or a particular patient type, working without direct supervision.

- The postholder will ensure their own personal development and participate in the development of others

- To supervise, educate and assess the performance of physiotherapy students to ensure the standard of practice and teaching meets the degree standard

- Undertake evidence-based audit and contribute to research projects. Make recommendations to clinical lead/manager for service improvement

- Willingness to meet the travel requirements for the needs of the service.
