Job summary

Fylde and Wyre Community Mental Health Team has an exciting opportunity for a Band 6 Community Mental Health Practitioner to join the team supporting our complex emotional needs pathway delivering Structured Clinical Management. This post provides the opportunity for a forward thinking and enthusiastic individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.

You will have a working knowledge of Structured clinical management and be able to work with a complex range of emotional needs undertaking assessments, care planning and the risk assessment.

Fylde and Wyre CMHT works over 2 bases due to the geographical size of the catchment area. You will have a main team base but there is the expectation of support across both sites as required.

Main duties of the job

1. Working for the local CMHT, clinically supervised by the Personality Disorder Managed Clinical Network, to provide high quality interventions for service user's who have a diagnosis/present with difficulties associated with the diagnosis of Emotionally Unstable Personality Disorder (EUPD)

2. You will be aligned to work within a Community Mental Health Team (CMHT) providing Structure Clinical Management (SCM) for a caseload of service user's. You will facilitate and lead groups to support the recovery of service users.

3. Promote positive attitudes towards service users with complex needs by providing a responsive and non-stigmatising service, which will enhance inclusion and deliver better outcomes.

4. To undertake mental health assessments including risk assessments and be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.

5. Providing a communication structure between GP's, IAPT and secondary MH services in LSCFT, non-statutory service to ensure the highest possible standards of patient care 6. To provide advice and support for the CMHT Multidisciplinary Team to deliver the highest standards of care for service user's with a diagnosis of EUPD. This may include joint working at times.
