RMN - East Lancashire - Pennine Based at Ballenden House

Part Time - 22.50 hours per week

We have an exciting opportunity to offer a Band 5 Registered Mental Health Nurse to join our Older Adult Community Mental Health Team within the East Lancashire/Pennine locality, the team is based at Balladen House, Rawtenstall, overlooking the beautiful Rossendale Valley.

The service provides assessment and treatment to service users referred with a variety of mental health conditions within the community, including those identified with Dementia. The team is multi-disciplinary and works closely with other mental health services and teams.

Potential candidates need to have relevant Older Adult /Adult Mental Health community experience and have proven clinical skills within Community mental health services. We are looking for a candidate who has excellent clinical skills and knowledge, and a positive attitude to the care of Older Adults within the community setting.

Knowledge and skills around the role of a community nurse is essential in line with the current work around transformation and the move from CPA to Dialog+. It would be desirable for the candidate to have an understanding around, models and initiatives, underpinned by clear crisis/care planning and assessments.

The postholder will be required to provide personal or clinical care to male service users in situations where this may not be appropriate for female staff

Main duties of the job

The ability to manage risk, liaise and communicate effectively and have a working knowledge of current national and local drivers is essential. Candidates will be able to demonstrate resilience and the ability to manage the demands of a busy community team. Be able to offer management and clinical supervision to band 4 or 3 staff, and act as a mentor to support student allocation and activity.

There is a robust supervision and personal development structure in place via the service manager and the successful candidate will have development opportunities in house and externally.

To provide high quality care within secondary services to older adults with sever and enduring mental health problems.

To work autonomously within professional guidelines and the overall framework of team policies and procedure

To deliver secondary mental health services in accordance with NICE guidelines, within agreed activity levels and model, meeting required key performance indicators (KPIs).

To provide advice and consultation on client's care, to other clinicians and to non-professional carers.
