Job summary
NL&SC SPCMHT covers a vast footprint including Blackpool, Fylde & Wyre, Lancaster and South Cumbria. We are seeking applications from confident and enthusiastic professionals who are passionate about working in a perinatal service.
The post holder will support MDT therapy treatment plans, promote and empower our service users in their recovery, provide practical social support and support MH Practitioners with intensive support. The post holder will be expected to understand the principles of safeguarding and how this applies in the perinatal period.
We would welcome applications from both male and female applicants and although it would be an advantage there is no requirement to have previous perinatal experience.
Main duties of the job
To work as part of a team that provides perinatal mental health services, to positively promote recovery and social inclusion by focusing on the direct needs of service users, working across boundaries of care, organisations and roles
To provide support directly and flexibly to service users towards goals defined in his or her care plan.
To create specific care plans aimed at promoting social engagement, healthy living and independence.
Maintain information and knowledge of and links with community resources and actively support service users to use these resources, thereby countering discrimination and encouraging social inclusion.
Planning and organisation of activities, care plans and programmes of care in the Home setting, in collaboration with the service user and designated Crisis Practitioners and to assist in the assessments and "gatekeeping" process.
To be responsible for the compilation of an up-to-date database of opportunities available in an allocated life domain by detailed research and networking into the statutory, voluntary and commercial sector including specific searches for resources which are targeted at minority and under-represented groups. Information is to be recorded and categorised in such a way as to offer quick and easy access to others.
