Job summary
Preston East Community Mental Health Team within Lancashire & South Cumbria Foundation Trust has an exciting opportunity for a Band 6 Mental Health Practitioner to join their well-established team.The team provides clinical and practical intervention to adults with complex mental health issues. The team offers a range of management strategies and interventions. Applicants must demonstrate a flexible approach, evolving around the needs of patients, their families and carers.The role involves working as a mental health practitioner in a well established team, including acting as a keyworker and supporting a caseload of clients with varying mental health diagnoses. As a keyworker you will work closely with people in their own homes or in the community to help them recover from their illness and improve their general quality of life. This role includes assessing patients, providing care/support, developing risk assessments, collaborative care planning, medication management and physical health monitoring, managing patient documents. Alongside becoming an advocate for the people in your care, supporting and signposting them, so they can focus on their recovery and helping them live the life they would like to live.
Main duties of the job
We are looking for an enthusiastic person with excellent team working skills, who will promote teamwork, respect, integrity, accountability, excellence and compassion. We are looking for someone who is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience in accordance with the Trust values and your governing Professional body. You will work as part of a multi-disciplinary team providing assessment and therapeutic intervention in the community.
