Job summary

An exciting opportunity has arisen for an experienced Speech & Language Therapist with advanced clinical skills to be part of the established & expanding Community Neurological Rehabilitation Team (CNRT) in Central Lancashire. The Community Neurological Rehabilitation Team provides ongoing rehabilitation to patients within the Chorley, South Ribble and Greater Preston locality with predominantly Stroke and ABI diagnosis. The CNRT is a large , supportive, friendly team that values interdisciplinary working, striving to provide clinical excellence, lead by a dynamic team of Clinical Specialists including Speech and Language Therapy, Physiotherapy and Occupational Therapy disciplines along with Neuropsychology and highly experienced generic rehabilitation assistants. Our rehabilitation pathway is patient centred where functional goals are embedded within team practice and shape and inform the delivery of therapy. We provide therapy in a number of environments including clinics, patients' homes and other community settings. We use Ampcare, React2 and Tactus software to support patient therapy programmes and have access to N-ROL & Videofluroscopy clinics.

Main duties of the job

We are looking for an enthusiastic, versatile, innovative individual with a commitment to working within an interdisciplinary environment, who has extensive experience of working with patients with stroke and neurological conditions.

You will be responsible for the specialist assessment, treatment planning and management of individualised rehabilitation programmes for a wide variety of neurological clinical presentations and will be responsible for delegation to clinical staff and generic rehabilitation assistants as well as supporting them in the delivery of this rehabilitation.

You will have autonomy in managing your complex caseload and will take responsibility for ensuring commissioner lead service delivery and ensuring high quality, safe, patient-centred rehabilitation is provided throughout the team.

You will be part of a robust in service training programme as well as support for internal and external development within the field of Neurological Rehabilitation and leadership.

The post-holder will demonstrate clinical leadership and represent the CNRT to support the planning and service development within SLT and the wider MDT,

You will provide regular support and clinical leadership to the SLT's and the wider MDT, you will be supported by the ACP/Principle Practitioner to develop and expand your existing skills and knowledge.

Due to the nature of the role, it is essential that you are able to travel independently across the locality.

Essential

  • Degree or equivalent in Speech and Language Therapy
  • Current professional registration with HCPC as Speech and Language Therapist and responsibility for maintaining this registration
  • Masters module/post grad qualification in relevant field of clinical expertise (or working towards) / post-grad qualification/equivalent knowledge & experience to masters level

Knowledge

Essential

  • Understanding of clinical governance framework and implications for practice including audit and research governance
  • In-depth understanding of current NHS/government policy/Public Health/NICE guidelines on organisational change and the ability to adapt these appropriately to local service needs through publication of documents
  • Knowledge and appreciation of lone working risks - policy and procedure

  • Treatment of a broad range of conditions relating to clinical speciality
  • Demonstrable evidence of improving and developing service and practice
