Job summary
An exciting opportunity has arisen for an experienced Qualified Band 6 Mental health practitioner to join The Bay Frequent Attenders Service. This post will cover The Bay locality which will include Lancaster & Morecambe and South Cumbria areas.
Service users within the "Frequent Attender Programme" are high intensity/complex individuals with multiple issues and needs. They can be frequent attenders within the Emergency Department or high users of Section 136, they may also frequently utilise emergency services such as the police or ambulances. Many people will also be receiving care from Home Treatment Team or CMHT.
We need staff who are highly motivated and proactive and who are able to work assertively and empathically with people within the locality to encourage their engagement with multi agency services and support them within their recovery. This will involve working collaboratively with other agencies to aid engagement to meet the needs of the service user. The B6 MHP's will provide a comprehensive assessment and develop collaborative care plans with Service Users and other agencies and where necessary signpost to other services within the community. For those Service Users that are already known to Mental health services you will work collaboratively with the Care Coordinator to develop a robust plan of care that Multi agencies are involved with to support the person and prevent crisis.
Main duties of the job
o You will be a member of a team who provides comprehensive assessment, co-ordination and the delivery of a high quality mental health service for service users identified as a high user of services within the Urgent care pathway.
o You will help to co-ordinate the care of the service users, and assist in promoting the use of evidence-based practice through the provision of clinical advice and support; acting as a role model; and providing education and training where appropriate.
o You will be expected to have a sound understanding of the client group you will be working with, and be able to demonstrate a wide range of skill base examples and knowledge.
o You will need to be fully conversant with statutory and non-statutory services in Lancashire Care, and promote effective liaison, working relationships and operational links as appropriate.
o You will ensure your own safe and effective delivery of care to service users.
o You will provide support, information and education to family members, carers and other professionals.
