Job summary
An exciting secondment opportunity has arisen in the Patient Safety Team to support the team in the investigation and learning from patient safety incidents.
The Patient Safety Lead will support the Patient Safety Team, alongside two other patient safety leads, to implement the Patient Safety Incident Response Framework (PSIRF) and conduct PSII investigations. The post holder will also be required to work closely with the Assistant Director of Governance to support with all aspects of quality monitoring and learning from patient safety incidents, across all localities. The role is integral in ensuring the safety of patients and staff.
This role will be initially a 12 months secondment.

Main duties of the job
The Safety Lead will support the Assistant Director of Patient Safety and Head of Patient Safety to operationally lead the Trust-wide approach to safety improvement and learning. They will lead on the processes for Investigation and Learning to develop and improve the quality of investigations related to incidents and complaints, and in ensuring the subsequent learning is embedded across the organisation.
As a Corporate Investigations and Learning Specialist the post holder will be expected to take on the most complex and serious incident investigations; for example actual and attempted homicides, completed or suicides, cluster incident investigations as designated by the Head of Patient Safety.
The post holder will report to the Head of Patient Safety and will responsible for the line management of a number of administrative staff as designated by the Head of Patient Safety.
The post holder will work with a range of internal and external stakeholders to investigate, analyse and use judgement through the collation of evidence and findings from examination of records, review of best practice, policy and guidance and interviewing individuals. The outcome will be the production of a timely high quality report. In order to achieve that the post holder will be required to analyse and interpret complex clinical situations and arrive at well thought out risk assessed decisions in a short period of time
