Job summary

An exciting opportunity to join Lancashire and South Cumbria NHS Foundation Trust's to work in Physical Health Services within the Pennine Locality Network as a Band 6 Specialist Occupational Therapist within the Community Rehabilitation Team. The team includes Occupational Therapists, Assistant Practitioner and Community Rehabilitation and Health Care Assistants. We work with the wider multidisciplinary team and support long term rehabilitation with patients in their own home's. We attend Integrated Neighbourhood Team meetings weekly to support the health and well-being of Blackburn with Darwen residents using a multi-disciplinary approach with health, social care and voluntary sector partners. We work closely with our Intensive Home Support Service colleagues to support a patient remain at home support and this service operates 365 days a year, 8.00am - 8.00pm.

We are looking to appoint a dynamic, enthusiastic Occupational Therapist to join our service. The role will involve assessing patients, planning treatment and coordinating support and equipment to meet the patient's needs. The successful candidate will need excellent communication skills who is confident to work autonomously as well as part of a team.

Main duties of the job

To be responsible for the assessment, treatment, planning and delivery of evidence -based rehabilitation interventions.

Provide clinical triage and support the management of patient demand through the system-discharging appropriately.

To undertake comprehensive assessment of patients with complex presentations, using investigative and analytical skills, to formulate an appropriate hypothesis and to determine a diagnosis and appropriate management plan.

To establish individualised management and treatment plans, using clinical reasoning and utilising a wide range of evidence based treatment skills and options to formulate highly specialised interventions and programmes of care, which may include working with staff across organisational boundaries.

To lead/contribute to training and development programmes as appropriate to the knowledge and skills of the individual, delivering to other professionals both within and outside the NHS and service users.

To participate in the LSCFT PDR review process ensuring business priority objectives are met and a personal development plan is maintained and evaluated. To cascade the PDR process to departmental staff as appropriate.
