Job summary

We are looking for someone who: enjoys and works well in a team setting and is also able to work independently; is committed to working with older people with mental health needs and is able to work effectively in this demanding role.

As a Lead Professional, you will provide evidence based interventions, risk assessment and management to a broad range of people with functional and organic mental health needs. You will be responsible for the assessment, planning, implementation and evaluation of care needs and be responsible for the clinical management of a defined caseload. You will also assist with the triaging and response to incoming queries and concerns.

This integrated team provides a Memory Assessment Service, care for patients with more complex needs associated with both functional and organic illness, and Care Home and Hospital Liaison. The unique set-up of these integrated teams allows a seamless delivery of care for patients throughout all stages of their illness, and a collaborative approach within the team.

The team serves the population of the Furness Peninsula at the edge of the Lake District and has its base in Barrow-in-Furness. It is an established team with a friendly and supportive culture. There is a visible and supportive leadership team who are committed to clinical supervision and professional development.

Main duties of the job

  • Provide assessment, advice and treatment to people with Dementia and complex mental health needs referred to Community Services in accordance with care pathways for given specialism
  • Deliver interventions in accordance with care pathways for given specialism
  • Complete timely risk assessments, formulations and management plans
  • Work closely with other professionals within and outside the Community Team
  • Actively support the Team Leader in the achievement of KPI / CQUIN and related County Council targets, and maintenance of registration with regulating bodies
  • Takes the lead on specific objectives as directed by the Team Leader, including supporting any changes to service delivery.
  • Provide supervision and direction to junior staff within community services, including B5 colleagues.
  • Contribute to the screening of referrals and allocation to service line
  • Support a culture of ongoing learning and development on an individual and team basis to ensure high standards are maintained and, where appropriate, contribute to the delivery of training as appropriate
  • Actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and code of conduct.
  • Provide leadership to junior colleagues acting as a positive role model
