Fixed Term - 12 Months

Rossendale & Hyndburn Community Mental Health Team are seeking to appoint a Band 6 Community Mental Health Practitioner. This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships. You must be a registered mental health practitioner, with excellent leadership and clinical skills.

You must have significant post qualification experience in clinical work with service users experiencing a range of mental health problems

You need to be able to work as part of a multidisciplinary team and have experience of assessing, planning and implementing and reviewing care needs.

You will also need to have a working knowledge of the care programme approach, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.

Main duties of the job

To undertake mental health assessments including risk assessments within a multi-disciplinary assessment team. Working closely with the Consultant Psychiatrist, thus ensuring that there is a multidisciplinary team approach.

To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.

To provide a fast, responsive service to assess individuals needs for access into mental health services.

Providing a communication structure between GPs , IAPT and secondary MH services in LCFT, non-statutory service to ensure the highest possible standards of patient care.

To provide mentoring and support for others.
