Job summary

We have an exciting opportunity for an experienced Paediatric Nurse seeking to undertake an interesting and highly rewarding clinical and team leadership role.

This team leader role will take responsibility for the clinical leadership and management for the Children's Continuing Care & Assessment Team (CCCAT).

The CCCAT provides Health Needs Assessments (HNA) for Children & Young People (CYP) who meet the criteria for continuing care - on many occasions this facilitates the transfer from hospital to home. The assessment requires significant liaison with families and professionals across the region as well as Integrated Care Boards (ICB) to ensure an accurate assessment of need is established and then can be commissioned. Once the level of support is identified the team is involved in the regular review of the CYP responding to changes in need and supporting them in the community until they no longer require the service or transition in to adult care. The service also has a function in the provision of consumables and equipment.

The post holder will ensure a high quality evidence based service clinical care pathways are delivered utilising available resource effectively in a safe and supportive manner when meeting the needs of CYP and their family.

Main duties of the job

You will be responsible for the clinical governance, quality improvement and performance management of the team and will provide management supervision to team members and ensure that staff within the team have opportunities for continuing professional development.

To provide clear line management within the team, including the full range of first line management duties, including approval of annual leave, special leave, training, undertaking PDR, investigating and responding to complaints and dealing with first stage disciplinary and grievance issues.

To support training and development needs associated with the specialist areas of clinical practice within the Service Line.

To work in collaboration with Service Line Clinical lead, Professional Lead and SIM to support audit and research opportunities, implementing projects within the team.

To work as part of a locality leadership and management team with in the Service Line.

To oversee comprehensive assessment of children and young people with complex presentations.

To oversee the establishment of individualised management and treatment plans, using advanced clinical reasoning and utilising a wide range of evidence based treatment skills and options to formulate specialised interventions and programmes of care.

To promote health and reduce inequalities for children, young people, families and communities, so that they may be able to maximise their potential in terms of health, education and social well-being.
