An exciting opportunity has arisen to work in Kendal as part of our progressive, integrated OA CMHT as lead memory nurse across South Lakes. We are looking to recruit an enthusiastic Mental Health Nurse to work with us to continue to develop our Memory Assessment Team.

As a member of the South Lakes Memory and Later Life Service, you will use your mental health nursing skills and knowledge of evidence based practice to provide assessment and treatment for older adults with a range of complex mental health issues, working closely with multi-disciplinary colleagues to deliver recovery-based care pathways. You will primarily be working with people who have cognitive impairments across a range of care pathways, from early memory needs to established dementia, and living in various community settings. You will use specialised memory and nursing assessments to assist with the assessment, planning and implementation of care for diagnostic purposes and post diagnosis support.

We use a range of comprehensive assessment tools including the ACE3 to support patients and their carers in the diagnosis of memory impairment in the Memory and Later Life Service, and there will be many opportunities to develop and consolidate your skills.

Main duties of the job

You will also support and supervise band 5 nurses, AP's, support workers and student nurses in the team and you will provide advice on nursing and memory perspectives within the MDT.

  • You will be a member of the multi-disciplinary OA CMHT within the memory assessment service team and as an independent specialist practitioner you will ensure the effective assessment, planning, consultation and formulation towards diagnosi
  • To have a specialist knowledge and understanding of strategies and interventions in the treatment of dementia
  • To be proficient in referring to other agencies within the service or referring to other services or agencies where necessary
  • To confidently identify risk factors and action accordingly
  • Providing a communication structure between GP's and other services in LCFT, non-statutory services, as a means of ensuring highest possible standards of patient care
  • To provide mentoring and clinical supervision to other staff and trainees on placement
  • To actively promote the Trust values within the OA CMHT service and in accordance with the NMC code of conduct
  • To assist the team leader in ensuring that leadership within the MAS team provides effective assessment, formulation, planning and monitoring of care given to patients and their carer's
  • You will also be expected to actively contribute to the ongoing development of the OA CMHT and MAS.
