Job summary
An exciting opportunity to join Lancashire and South Cumbria NHS Foundation Trust to work in Pennine Physical Health Services as a Band 6 Specialist Occupational Therapist within the Community Rehabilitation Team. The team includes Occupational Therapists, Falls Specialist OT, Technical Instructor's and Community Rehabilitation and Health Care Assistants.
The successful candidate will support patients and their families after a recent discharge from an acute or community hospitals. They will promote continuity of care into the community setting working closely with the Home First Service. They will also work with the wider multidisciplinary team to support long term rehabilitation pathways for patients in their own home's to support increase or maintain their independence. The role will involve assessing patients, planning brief treatment plans and coordinating support and equipment to meet the patient's needs.
Community Rehabilitations Service attend weekly Integrated Neighbourhood Team meetings to support navigate referrals to the correct services with health, social care and voluntary sector partners.

Main duties of the job
- To be responsible for the assessment, treatment, planning and delivery of evidence -based rehabilitation interventions.
- Provide clinical triage and support the management of patient demand through the system-discharging appropriately.
- To work as part of the Community neurological service line and be responsible for assessment and management of a clinical caseload of patients with complex needs.
- To undertake comprehensive assessment of patients with complex presentations, using investigative and analytical skills, to formulate an appropriate hypothesis and to determine a diagnosis and appropriate management plan
