Job summary
The Liaison & Diversion service operates an All Age (from age of criminal responsibility - 10yrs), all Vulnerability model based on the National Liaison & Diversion operating model produced by NHS England.
The Liaison and Diversion Specialist Practitioner is someone with a clinical qualification and experience and knowledge of Liaison and Diversion and/or with specialist knowledge and experience of working with groups likely to be disadvantaged by their involvement in the justice system.
Specialist Liaison and Diversion Practitioner, a professionally registered practitioner to work within the Criminal Justice Pathway, providing assessment of Mental Health, Learning Disability, Complex Health and social needs and Capacity, liaison, referral and guidance for persons who have come to the attention of the Criminal Justice Agencies.Important: This post is requires successful candidates to undergo Level 2 Vetting from Cumbria constabulary from which you will be working, of which clearance is essential in order to fulfil the role.
Main duties of the job
- To support junior colleagues and provide high level supervision and support with clinical practice.
- To oversee case management of caseloads within the team, offering regular guidance and support to junior staff. To delegate daily tasks on shift
- To deputise for the team manager, supervising staff on shift. .
- To undertake screening assessments of Adults and children aged 10 years and over, within the community or criminal justice setting.
- Obtain valid consent for healthcare investigations within a criminal justice setting ? Utilising identified all vulnerability and age appropriate screening tools.
- Assess risk to self and others, using five 5P Risk Formulation.
- To gather pertinent information from a variety of sources, including the service user, carers and other Health and Social Care and criminal justice Professionals.
- To identify unmet vulnerabilities and needs that may include some of the following:
- Mental Health concerns o Capacity issues o Acquired Brain Injury
- Learning difficulty/Disability o Accommodation issues o Educational needs
- Speech Language and Communication needs o Financial difficulties including debt
- Substance Misuse o Autism o ADHD
