Job summary
Fylde Coast Urgent Care Pathway are seeking a skilled Registered Nurse/Social Worker to join us in the role of Homeless Link Worker. This is a new post so will provide an excellent opportunity for a practitioner who wishes to support service development, has a positive outlook and always puts the service user first.
Main duties of the job
Link working and developing relationships with other services is essential and it is expected that the practitioner will be autonomous and proactive with a special interest in the unique needs of the homeless population in the Fylde Coast.
You will be integral to the Urgent Care Pathway and a valued member of a service that always seeks to provide resolution.
