Job summary

An opportunity has arisen for an experienced, enthusiastic and skilled Community Mental Health Nurse to join our busy Community Mental Health Team for Older Adults.

We are looking for someone who: enjoys and works well in a team setting and is also able to work independently; is committed to working with older people with mental health needs and is able to work effectively in this demanding role.

The team serves the population of the Furness Peninsula at the edge of the Lake District and has its base in Barrow-in-Furness. It is an established team with a friendly and supportive culture. There is a visible and supportive leadership team who are committed to clinical supervision and professional development.

Main duties of the job

As a Care Coordinator, you will provide evidence based interventions, risk assessment and management to a broad range of people with functional and organic mental health needs. You will be responsible for the assessment, planning, implementation and evaluation of care needs and be responsible for the clinical management of a defined caseload. You will also assist with the triaging and response to incoming queries and concerns.
