Job summary

This is an advanced clinical role for the team which will support all the leadership aspects of the service including project and service development work, the mentorship and teaching of junior clinical team members and team management development opportunities, and deputising for the team leader when required. You will be expected to participate in the development and delivery of local diabetes care strategies and guidelines. Having a non-medical prescriber qualification will be required for this advanced role, however there will be opportunity to develop and undertake the V300 qualification.

Working with the DSN team is varied, busy and exciting. The team is responsible for delivery of specialist care for patients with diabetes. We run nurse led clinics providing assessment, treatment plans and follow up appointments in face to face community clinics or virtually using digital video platform or telephone. We also visit patients who are housebound in their own home if required.

Main duties of the job

We provide support to local mental health establishments, and work with many people with learning difficulties and their support teams. We provide support and training to District Nursing teams and their service users and residents in care homes who require specialist input.

We also offer advice and support to the hospice during end of life care. The team provide regular accredited diabetes education sessions for people with Type 1 diabetes. We also run education sessions for other health care professionals within primary care. We are forward thinking with regards to the fast moving digital changes and service developments for people living with diabetes and we have a number of digital platforms that we utilize to support our patients.

The post is currently based with the Diabetes Specialist Nursing Team at the Minerva Health Centre in Preston.
