Job summary

Providing holistic needs based assessment, intervention and stabilisation of clients during a crisis over a 96 hour period with the aim of avoiding unnecessary admission to hospital.

To provide holistic, needs based assessment of clients requiring step-up or step down routine intermediate care assessment.

To refer on to and co-ordinate services.

To in-reach into the Emergency Department, Urgent Care Centre and the Medical Admissions Unit as required

Main duties of the job

We are looking for an enthusiastic and motivated NMC registered nurse, interested in acute, client centred care within the community, to join the Blackburn with Darwen Intensive Home Support Service (IHSS) . It is an integrated multi-disciplinary team of Occupational Therapists, Physiotherapists, Nurses, Assistant Practitioners, Complex Case Managers and Support Workers.

This role involves assessing patients within their own home environment to reduce avoidable admissions and support people within the community at times of crisis due to deterioration in health. The IHSS service operates 8.00am - 8.00pm 365 days a year.

You will need excellent communication skills and have experience of teamworking. The role will involve delivering excellent, evidence based nursing care and involve a holistic approach to emphasising the elements of rapid response, intravenous therapy, Complex Case management, recuperative and rehabilitative care.

The Intensive Home Support Service A multidisciplinary team comprising Occupational Therapists, Physiotherapists, Nurses, Assistant Practitioners, Community Rehabilitation Assistants and administrative support, providing a 365 day a year service, 8am to 8pm, within the community and input as an in-reach service until 8pm in the Emergency Department, Urgent Care Centre and Medical Assessment Unit of Royal Blackburn Hospital to avoid admissions and facilitate supported discharge.

The team works with other community professionals such as GPs, District Nursing, Community Matrons and Care Managers to prevent avoidable hospital and emergency respite care admissions and provides:

  • Comprehensive multi-disciplinary assessment of needs including appropriateness for Community Rehabilitation and admission to Residential Intermediate Care Services
  • Specialist clinical and therapy assessments, interventions and rehabilitation programs up to 96 hours
  • Specialist equipment and aids to daily living to maintain safety and promote independence in the patients home
  • Co-ordination of crisis support across boundaries of care with partners working for patients requiring assistance to maintain primary personal care and social care needs
  • Patient education and promotion of self-management of the patients condition
  • An admission avoidance service for the acute trust in the evenings, at weekends and on bank holidays.

Key Relationships Maintain effective relationships, with other service teams and managers, from teams co-located within base, Third Sector Organisations and in both Primary and Secondary Care.

Key Responsibilities Responsibility for patient Care (including monitoring, diagnostics and investigations).To provide a holistic assessment in line with the single assessment process, to identify areas of concern and develop appropriate care plans.

To plan programmes of care to promote health gains, manage chronic and acute illness and maximise independence with the client group across a variety of settings.

To ensure clients receive timely intervention from the most appropriate professional/service

To develop and maintain skills and knowledge to assess the physical psychological and social needs of the client group, instigating therapeutic treatments based on best available evidence in order to improve health outcomes.

To interpret the available information and make an overview assessment of the clients health and social needs within the framework of the single assessment process, identifying potential risks to their future health and well-being.

To use clinical skills and knowledge to identify changes in the patients condition, and to prioritise patients needs in order to provide appropriate and timely interventions and/or referrals.

To instigate diagnostic tests, investigations and therapeutic treatments and/or refer onto other healthcare professionals appropriately.

To evaluate the effectiveness of clinical interventions in meeting previously agreed goals, negotiating with client and other members of the multi-disciplinary team where appropriate and making any necessary modifications to treatment plans.

To counsel patients and carers in relation to prescribed treatments to ensure safety understand and concordance.

To administer medications as prescribed (where appropriate), and monitor their effectiveness according to Trust policy and procedure and professional guidelines.

To liaise and communicate effectively with other professionals involved in medication management and prescribing.

To ensure the provision of planned interventions necessary to promote health gains, manage illness and reduce the risk of complications and deterioration of the patients condition.

To identify opportunities and provide health education and promotion for patients and their carers.

To identify opportunities to promote and improve the patients ability to self manage their illness or condition wherever possible.

To ensure that the care provided and services delivered are in line with Trust policies and procedures, and clinical governance framework.
