Job summary
The Crisis Resolution Home Treatment Team has an exciting opportunity for a mental health practitioner to join the North Mersey team. The Crisis Resolution Home Treatment Team operates on a 24/7 basis providing support for service users and families across Liverpool and Sefton area.
CRHT support people through a short term mental health crises by providing intensive treatment and support in their home environment as an alternative to inpatient care. The team can also help facilitate early discharge from hospital to provide home treatment. The team consists of Psychiatrists, Psychologists, Social workers, Mental health practitioners, occupational therapists and support workers.
The post holder will be expected to work across weekdays, weekends, and nights in accordance with the service need
Main duties of the job
To provide high quality, evidence based care within our Mental Health Care Division for service users and their carers within inpatient and community services.
To work in a socially inclusive way and in line with the recovery model.
To function as named nurse/care coordinator to service users undertaking assessment, planning and evaluation and review of care, within a recovery focused model of care.
To participate as an active member of the multi disciplinary team.
To provide advice, expertise and leadership within the team.
To supervise care for service users presenting with complex and high risk needs.
To work autonomously, reaching clinically appropriate decisions based on information received and using the knowledge and experience of the multi disciplinary team.
To be qualified to act as a mentor for students.
