o To contribute to the assessment and care management process within a multi- disciplinary team
o To make service users aware of financial contributions, to provide information enabling service users to make an informed choice.
o MDT working in both community and hospital settings (The Royal Liverpool Hospital Frailty Unit, Longmoor House Aintree Hospital, Liverpool Innovation Park).
o Assessing and commissioning packages of care and residential and nursing placements
o Hospital discharge planning

Main duties of the job

To follow social work code of conduct and follow all in house procedures, and confidentiality, GDPR etc.

- Recognised qualification in Social Work at degree level
- Current SW England registration
Knowledge

- Experience as a Social Worker in a health/social care setting.
- Good working knowledge of legislative guidance and policy frameworks including but not limited to:
- Care Act 2014 Mental Health Act 1983/2007 Mental Capacity Act 2005 Continuing Health Care Interagency Adult Safeguarding policies and procedures
- Knowledge and application of professional standards, codes of conduct and SW England standards of proficiency.
