Job summary
An exciting opportunity has arisen for Advanced Clinical Practitioners (ACP) to work as a Community Matron within Mersey Care NHS Trust, within Sefton Place, based in the South of the area.
You will be responsible for the delivery of proactive caseload management and provide first contact care in response to acute long-term conditions and acute episodes of illness for patients in their own homes and CQC Registered care homes.
The Community Matron is a key role in the Community Services that operates across South Sefton. This service combines the District Nursing service with trainee Advanced Clinical Practitioners and Community Matrons.As a senior clinician, you will demonstrate a high level of professionalism and provide clinical leadership within the Integrated Community teams.
Main duties of the job
As an autonomous practitioner the post holder will have an advanced clinical role with responsibility for assessing, planning, managing and co-ordinating the care of people, in their own homes and care home settings and with highly complex needs and long-term conditions within a defined caseload.
The post-holder must be educated to MSc degree level in Advanced Clinical Practice with relevant experience.
