Job summary

An exciting opportunity to has become available in the Healthy Knowsley Service, within our Children Families Team. This is an opportunity for someone who is passionate about reducing health inequalities and increasing health aspirations.

This will be an opportunity to provide in person interventions to families at risk of unhealthy weight. The postholder will engage with school communities to identify and enrol eligible families.

The postholder will maintain a caseload of children and families, under the direct/indirect supervision of a Senior Health Improvement Practitioner. They will deliver interventions that increase knowledge, skills and confidence to adopt health promoting behaviours. Healthy lifestyle messages will be delivered through activity based learning with an emphasis on being active, learning and having fun as a family.

The post holder will have a good knowledge of factors that impact overweight and obesity in children. They will have experience of facilitating engagement in health related activities for families. They will be skilled at adapting physical activity based sessions to ensure engagement.

Main duties of the job

The post holder will work as part of the Healthy Knowsley Service to support families and children to make sustainable changes to reduce health inequalities across the Borough. This will include the delivery of high quality interventions that: provide a positive experience of physicalactivity; increase activity levels in the whole family; increase knowledge of healthy eating messages; improve cooking skills and support sustainable changes in health related behaviours.

Interventions will be delivered in school and community environments and virtually.

The post holder will maintain accurate, clear, concise and contemporaneous records as per trust policy and communicate information to members of the Team as necessary.

The post holder will provide concise handover and information updates to other members of the Team as required.

MAIN RESPONSIBILITIES1. Meet with families to assess lifestyle risk factors and support them to develop relevantlifestyle improvement initiatives.2. Provide support, motivation and development of appropriate action plan for eachfamily.3. Carry out review meetings to assess progress and provide long term motivation andguidance.4. Deliver healthy lifestyle programmes to school communities.5. Deliver multi component interventions to families incorporating goal setting, physicalactivity, healthy eating and cooking skills.6. Monitor attendance and achievements of clients on courses or activities recording anddocumenting as appropriate7. Organise and facilitate groups and/or individuals to develop self-care skills.8. Whilst working with families and schools, identify barriers and possible solutions tomaking sustainable lifestyle changes, reporting these back to the Team Manager.9. Support the development of new programmes within the Healthy Knowsley Service.10. Support the development of new lifestyle initiatives which will offer peer support toclients trying to maintain lifestyle changes.11. To contribute to evaluation/research initiatives designed to generate data regarding theeffectiveness of the service.13. Participate in community events and meetings as required.14.
