Main duties of the job
Our Peer Support Workforce is expanding and we are looking to recruit Senior STR lived experience support workers to work in Criminal Justice Liaison and Diversion Reconnect Resettlement service.
The purpose of the RECONNECT care after custody service is to provide a seamless transition of care from custodial setting (either convicted and sentenced to a period of imprisonment or remanded to custody prior to conviction or sentence) to community based healthcare services, thereby safeguarding the health gains made in custody.
For many people, release from prison is a crisis point. They are leaving a secure environment where their needs (health, housing etc) are met, and moving to an environment where, not only are they responsible for themselves, however they may have interrupted care pathways and are often without accommodation. They move from a supported and regimented environment to one with little or no support and uncertainties
This post is subject to an enhanced check with adults barred list Disclosure and Barring Service (DBS) Check and police/prison vetting which will be considerate of any offending history
