Job summary

This is an exciting opportunity to join the Specialist Support Team (SST) - Greater Manchester as Band 5 Nurse Practitioner

Main duties of the job

We have developed a model of care and support that provides specialist health services to adults who have a Learning Disability and/or Autism and who present a risk of offending or who have a history of criminal conviction as well as individuals who are described as having complex challenging behaviour. The service is based on the principles that underpin the Transforming Care initiative; that services for people who have Learning Disabilities should be configured around and with local communities.

The team will also work to enhance the discharge process, ensuring that people who do need in-patient care stay for only for as long as they need it. The SST will provide expert care and wrap around support to individuals and their support teams on discharge from hospital and work with community providers to ensure the correct training and support is available. The team will also provide intensivesupport to prevent crisis and hospital admission. The service is underpinned by positive behavioural support principles.

The successful candidate will need to be a car driver andwilling to travel across Greater Manchester working with a number of partner agencies including health, social care and the Criminal Justice agencies. Hours are predominantly 9am-5pm Monday to Friday although some flexibility willbe required when delivering intensive support which may include some evening or weekend working on a planned/rostered basis.

Essential

  • Registered nurse Qualification - Learning Disabilities RNLD
  • Current NMC Licence to practice
  • Portfolio demonstrating evidence of learning and development

Knowledge/Experience

Essential

  • Experience of nursing care planning process for adults with Learning Disabilities
  • Ability to communicate effectively at all levels with other professionals and with people who may have significant barriers to understanding
  • Demonstrates good standard of verbal and written communication skills including basic IT skills
  • Basic Knowledge of national and local policy/good practice guidance relating to care of people with learning disabilities
  • Awareness of specialist assessment tools
  • Ability to work effectively within a multidisciplinary team
  • Able to plan and manage own workload
  • Development of care plans for community settings
  • Experience supporting a staff team to implement care plans and monitor effect and impact of these
  • Chair meetings, develop action plans and monitor progress

Desirable

  • Experience of working with learning disabled offenders
  • Knowledge of therapeutic/intervention approaches (e.g. a specific psychological therapy such as CBT, CAT, 70 in behavioural interventions, motivational interviewing, IABA training etc.)
  • Knowledge of learning disability and related forensic issues and knowledge of current policy regarding these individuals
  • Demonstrate experience at pre-registration level of working across a wide range of clinical areas
  • Knowledge and experience of the discharge pathway from secure services to independent living in community settings
  • Experience of risk assessment and risk management planning in community settings
