An exciting opportunity has arisen for a Full time Band 5 Nurse Practitioner to work within the St Helens Later life and Memory Service community Team.
The hours for this post are 37.5 hours per week
The operational hours are Monday to Friday 9am - 5pm and we are based on the Hollins Park site. The post will involve travel across the borough.

Main duties of the job
The LLAMS service is an accredited service which provides the comprehensive assessment and treatment of people with dementia; The team has 3 areas; assessment, memory for those with less complex needs and who are non CPA and community for those whose needs are more complex and are on CPA .The team is flexible in its approach to meet the changing demands and there is an expectation that you will work in any area, however this role at this time is within the Memory Team
The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care within a multi-disciplinary environment within the framework of Effective Care Co-ordination (ECC).
During preceptorship, the postholder will carry out range of duties under the supervision of a manager or preceptor.
Following the successful completion of a comprehensive preceptorship the post holder will be able to carry out the full range of duties defined within the job description.

Act as Named Nurse as required
Ensure that the nursing practices of the clinical area comply with the Trusts policies and procedures
Participate and assist in setting standards of service user care based on sound research findings.
Participate and assist in systematic monitoring and evaluation of nursing practice within clinical area
Carry out nursing care based on the principles of individual care programmes/nursing process in accordance with specialist integrated Care Pathways.
Assist in the development and implementation of integrated care pathways, including appropriate service user assessments and models of care.
Support the implementation and maintenance of team working
Participate in process of potential new admissions to the service, including the assessment of such individuals in accordance with the agreed care pathway and completion of quality reports where appropriate.
Develop therapeutic relationships with service users and carers.
Participate in case supervision to monitor standards of service user care and documentation.
Participate in regular clinical supervision as per Trust policy.
Participate in managerial supervision.
Ensure the ward environment is conducive to promote service users health, safety and well being, thereby preserving and respecting each service users personal dignity, privacy, religious and cultural beliefs.
Incorporate health promotion and health education into your practice.
Ensure record keeping is in accordance with Professional and Trust standards.
Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code.
Work in a professional and collaborative manner with the Multi-disciplinary team. Participate and promote positive intra agency working.
