Job summary

An exciting opportunity has arisen within the District Nursing Service; we are seeking to appoint a highly enthusiastic and motivated individual to join the service as a Health Care Assistant. Successful applicant would work under the direction of the district nursing Team leader supporting the provision of high quality holistic nursing care to patients. The Health Care Assistant will work without direct supervision on occasions providing care to patients in their own homes and other community settings, as delegated by the District Nursing Team leader following appropriate training and supervision to ensure clinical competence and safe effective care.

Operational hours of the service are 24 hours a day 7 days per week 365 days per year

Main duties of the job

To work as part of the Integrated Neighbourhood/ Locality team under the direction of the district nursing Team leader, supporting the provision of high quality holistic nursing care to patients, 24hours 7days a week

The Health Care Assistant will sometimes work without direct supervision providing care to patients in their own homes and other community settings, as delegated by the District Nursing Team Leader following appropriate training and supervision to ensure clinical competence and safe effective care, but will also work with the trained nurse providing substantial input in palliative care

To support other health care professionals in the provision of high quality care in the locality population.
