This new position is an exciting innovative post for a Senior Nurse Practitioner, to work within the East Warrington Primary care network (PCN) to support children and young people who are experiencing mental health difficulties.
The East Warrington PCN provides primary care services across three GP practices.

- Padgate Medical Centre
Mental health is as important to Childrens and Young People safety and wellbeing as their physical health. It can impact on all aspects of their life, including their educational attainment, relationships, and physical wellbeing.Across the East Primary Care Network, we are finding more children are presenting to the Practices who are struggling with their mental health and wellbeing, and therefore we are building a unique team of people with a range of skills to provide a children and Young People's mental health and wellbeing service across East Warrington's community.
You will work alongside a Children's Care Co-Ordinator, a Children and young person's wellbeing Practitioner, a Health and Wellbeing Coach and a Social Prescribing Link Worker. The role requires collaborative working with the East Warrington Primary Care Network clinical teams and mental health services to shape, design and deliver an excellent holistic service for our children and young people.
Main duties of the job
The successful Senior Nurse Practitioner will provide Mental Health Assessment, inclusive of Risk assessment and Safety planning and person-centred Care planning to the Children and Young people referred to the service.
To provide high standards of evidence based client centred care.
To provide case management, clinical assessment and interventions to children, young people and their families with complex mental health needs.
The role will include direct and indirect client work as well as consultation and liaison with other professionals around the specialist clinical area of CAMHS. The post-holder will be engaged in all levels of clinical response from urgent assessments to lower priority cases.
The post holder will be expected to work as a member of the CAMHS Multi-disciplinary team (MDT).

- To accept referrals of children and their families, allocated by the Specialist Child & Adolescent Mental Health Services multi-disciplinary team process.
- To assess, plan, implement and evaluate interventions that are agreed with the child and caregiver in accordance with best practice. Ensure comprehensive risk assessment of children, young people and families and that risk management plans are in place and documented.
- To review plans on a regular basis and discuss in clinical supervision any issues that needs addressing.
- To liaise with other agencies in terms of information gathering and sharing.
- To inform local child protection co-ordinator and senior clinicians of any concerns regarding protection issues.
- Use audit/outcome measures to measure change/quality in accordance with Specialist CAMHS procedures.
- To organise own caseload and work schedules in agreement with Senior Nurse/Clinician.
- To see families/children in the setting most appropriate for effective and efficient care.
- Undertake clinical procedures, including the measurement of physical characteristics relevant to their treatment plan e.g. blood pressure, weight etc.
- Monitor and record/report effects and side effects of any other prescribed psychotropic medication.
- Engage and support service users and carers utilising evidence based psychotherapeutic principles.
- Prescribe and deliver evidence-based psychotherapeutic interventions.
- Act as knowledge resource in specialist area of practice to other team members and other agencies as required.
- To ensure co-ordinated service user centred care with a proactive role with other involved professionals.
