Job summary

Are you a forward thinking practice nurse, well versed in the national agenda for practice nursing and looking for an exciting new role? An opportunity has arisen with the Community Division of Mersey Care Foundation Trust for a Team Leader for our Practice Nurse Development Team. This unique team, the only one in the country, consists of experienced Practice Nurses who provide mentorship and support, including acting as cytology mentors, to develop Practice Nurses in GP surgeries across Liverpool.

Main duties of the job

The expectation will be to focus on the strategic development and operational management of the service and lead and manage a team of highly skilled and knowledgably clinicians through change, service development and the development of clinical leadership. You will work closely with teams across the community division, services within other divisions and with external partners to deliver specialist care, training and develop new ways of working.

It is an essential requirement that you have specialist knowledge and experience in practice nursing with a proven track record of clinical excellence and leadership. Evidence of this experience will be required and will need to be demonstrated throughout the recruitment process. This post will require an individual who is enthusiastic, self-motivated and passionate about person centred quality care delivery

Job responsibilities

1. To lead a team to provide highly specialised clinical care and advice and support to service users and the wider healthcare team this will include clinical assessment, diagnostic skills, non-medical prescribing and implementation of management and treatment plans.2. Be responsible for the delivery of the service within the different care delivery setting3. Work cooperatively and collaboratively with other services/stakeholders to enable service users are treated in the most appropriate setting and at the centre of the care delivery process4. Ensure that the team provides a high-quality service to its clients by providing caseload management support and clinical supervision.5. Ensure that teams work proactively in order to deliver anticipatory and maintenance care, providing a responsive service to service users with both planned and unplanned care needs.6. Maintain clinical credibility by providing clinical care and supporting members of the team within the clinical environment.7. Participate in the development of caseload management across the local health economy.8. Provide leadership and mentoring to those staff developing into a caseload management role.9. Acts as an advocate and champion for patients/clients in a variety of forums and professional groups and challenge attitudes and behaviour.10.
