This is an exceptional opportunity to be part of an expanding the Telehealth team that uses Telehealth Technology to prevent hospital admissions and improve wellbeing and general health .

Working in partnership with Cheshire and Merseyside and other health and social agencies, we have delivered this programme to over 10,000 patients .

A nursing NMC registration is essential for this post and experience in managing patients via remote monitoring and telehealth would be advantageous

Main duties of the job

This role will provide clinical leadership in the safe deployment of Telehealth projects,The role will provide clinical leadership to support digital opportunities, navigate barriers and work with teams on successful implementation, combining knowledge of patient care, clinical pathways, digital approaches, and change management to effectively address the digital needs of healthcare professionals and patients to promote safe, effective, and efficient use of digital in clinical settings. The clinical lead is to work with clinical teams, stakeholders, and corporate services across the organisation and beyond to help understand and implement clinically led digital monitoring that makes the most difference to patient care, clinical pathways, and clinical workforce.The role will provide clinical leadership and quality improvement expertise supporting, facilitating, and guiding the division, emphasizing, clinical safety, quality improvement, risk management, and clinical governance methodology to deliver the perfect care vision using appropriate tools, systems and processes that support the Trust's Quality Improvement Strategy.The post holder will work closely with the Head of Service and clinical service manager and clinical governance team sharing a range of quality improvement approaches to ensure the safe implementation of digitalized clinical pathways adhering to clinical governance policies and deliver high-quality and safe patient care in all settings across the Division.

Job responsibilities

1. The post holder will support and transform the care delivery by promoting and supporting the implementation of end-to-end digitalised virtual wards and remote monitoring clinical pathways adhering to the trusts quality improvement (QI) strategy, which aims to provide safe and effective care and promote a positive patient experience. This role will also provide leadership to members of the workforce who find digital technology challenging or are resistant to change.2. To work as an autonomous practitioner, provide organization-wide clinical leadership in digitalised patient care, liaising with all members of the multidisciplinary teams, including external organizations and stakeholders, to provide clinical expertise regarding digital initiatives from business case development, risk assessment, clinical system configuration, build, roll out, safe implementation and evaluation and transition to Business as Usual (BAU) services.3. Ensure organisational learning is embedded across the Division; learn from good practice when things are unexpected, comply with and contribute to CQC requirements, investigate incidents, lead clinical audits, provide constructive feedback, lessons learnt, and QI action plans with the community division to improve shared learning and patient safety4. Ensure practice is consistent with the Trusts objective of a Just and Learning Culture and supports and promotes the delivery of excellence in patient and carer care provision.5. Working with the Deputy medical director, Head of Service, clinical service manager, senior operational and nursing quality leads, clinical governance team and project team to develop robust clinical governance and dynamic quality improvement culture, ensure involvement in redesign processes, and work closely with clinical staff.6. Lead and create the Standard operating procedures (SOPs) and ensure clinical governance sign-off process and approvals are undertaken according to the Trusts protocols and guidance. Reviewing SOPs annually and as per changes in processes and clinical pathways. Implementing clinical policies and procedures, conducting clinical audits of protocols and integrated pathways to improve quality and key performance indicators using evidenced based practice/ research to facilitate change in practice and improves clinical outcomes, meeting the needs of patients and carers7. Leads the development, implementation of and audit of protocols, policies and integrated pathways using current evidenced based practice/ research that facilitates change in practice and improves clinical outcomes, meeting the needs of patients and carers. Ensure clinical research is conducted in line with Mersey Care NHS Foundation Trust protocols and liaison with the research team, ensuring that the research has had ethical approval.8. Develop and implement strategies to build capacity and capability in quality improvement methodology that supports the implementation of QI strategy via lessons learnt, implementing and evaluating clinical and safety audits, and leading and contributing to clinical safety huddles9. With the support of the Head of the service and Clinical service manager, the post-holder will provide clinical leadership and lead projects in telehealth to ensure clinical safety and03/5/23governance10. The role will support the Head of Service to provide professional leadership and exemplify positive behaviors and attitudes of the team, supporting the aim that staffs demonstrate the right values, knowledge and skills required for quality improvements and digital innovation based on evidence-based practice11. Leading the clinical process mapping, delivery, monitoring, benefits realization, and evaluation of assigned clinical digital projects and Programmes within the timeframe, providing feedback to stakeholders, and contributing to the digital transformation of place-based monitoring pathways across the region.12. The role will provide leadership, create a continuous improvement and excellence culture, promote high expectations and high achievement, attend system and clinical task group meetings, identify opportunities for future developments and review our compliance with required standards ensuring the safety of digital tools through clinical risk management approaches.13. Lead on mobilizing new clinical pathways in collaboration with external clinical leads, including virtual wards across the region, design staffing models, and plan staff profiles according to the clinical requirements to assist the finance team in costing and efficient budget management for the service. This includes ensuring a sound project plan & governance and assurance by identifying, appraising, mitigating, and managing risks and issues.14. Providing advice, guidance, and information regarding nurse-led digital remote monitoring care delivery model across the division15. Assess, appraise, and critically apply evidence-based clinical practice on QI.16.
