Job summary

Join our team! Fantastic opportunity for a Band 6 Community Mental Health Practitioner to join the St Helens Recovery Team working as part of the newly developed Relational Emotional and Strengthening pathway for adults who present with difficulties associated with personality disorder. We are a friendly and supportive team, and we would welcome applications from people with a core mental health profession who are passionate about offering compassionate, trauma informed and recovery orientated care to service users presenting with relational and emotional difficulties.You will work alongside experienced and supportive practitioners, providing NICE guidance interventions for service users who would likely meet criteria for a personality disorder diagnosis.There will be an expectation that you will either currently have the relevant training/experience required for this role or be willing to undertake this training.The team operates between the hours of 9am-5pm Monday to Friday. Our office is based at Harry Blackman House.

Main duties of the job

To work on the Relational and Emotional Strengthening (RESP) pathway. Main duties include:

  • Support the band 7 RESP pathway link worker to case manage complex clients
  • Carry a small caseload of clients for Structured Clinical Management (SCM)
  • Complete specialist RESP assessments
  • Attend specialist supervisions, meetings and training

Job responsibilities

Undertake personality disorder assessments and develop Living Life Well Crisis Care Plans

To maintain a small case load of RESP pathway clients (for assessment, socialisation or full programme) and support the Band 7 PD link worker role in delivering the RESP pathway.

To be flexible in supporting RESP pathway where the clinical need is, offering support to duty team with RESP pathway clients

To attend weekly RESP pathway consult meetings

To attend training programmes (as when required)

To support and provide training to other staff

To be active in managing the clients RESP pathway through the system, including in-reach work on the ward.

During admissions to liaise with the ward staff and Home Treatment Team and remain in contact with the service user throughout involvement with these service. Try to ensure the admission has clear goals, aims and objectives and take a lead role in supporting discharge/transition.

Facilitate a planned discharge. Organise an MDT meeting if necessary. Provide information regarding how to re-access services.

To advocate for clients with a personality disorder.

To implement the RESP pathway programme for borderline personality disorder

To work in accordance with the standard operating procedure for cluster 8

Formulate and implement appropriate treatment plans in line with RESP pathway model

Provide short term goal focused support / input to cases on the RESP pathway

Liaise with other professionals to contribute to the establishment of continuity of care across the service

To ensure that continuity of care is maintained and to support services to manage transitions between services in line with NICE guidance, including Section 117

To be able to provide consultation and training in personality disorder to other team members as required

Demonstrate an ability to formulate clients mental health needs

Agree to undertake additional training when required to facilitate role

To support the adherence of the RESP pathway.

EDUCATION AND DEVELOPMENT

Qualified Mental Health Practitioner e.g. RMN; or / Occupational Therapist with demonstrable post qualification and further training in Personality Disorder.
