Job summary

We are looking for an enthusiastic practitioner to join the Specialist Perinatal Mental Health Service. We offer full and part time working hours within 9am - 5pm Monday - Friday. Your main base will be Liverpool Womens Hospital.

You will be working within a specialist service that is committed to improving mental health care for women and families. You will be expected to fulfil the role of care co-ordinator as well as completing initial assessments. We utilise a flexible approach to the delivery of clinical care to best suit the need of mother and infant.

You will be expected to demonstrate meeting a set of key clinical competencies over a 12 month period to enable you to progress into the role of Band 6 perinatal mental health practitioner. You will be provided with support from other team members to enable you to develop your knowledge and skills to meet the given criteria.

Specialist training opportunities will be available as part of this role.

You will need to be able to travel across a large geographical area.

Main duties of the job

Act as care coordinator for a designated number of patients.

Formulate individualised care plans and risk assessments in collaboration with patients and carers where appropriate.

Participate in the triage process with support from senior staff members.

Participate in completing initial assessments for patients who have been accepted via triage process.

Liaise with other health care professionals and third sector organisations to share information and ensure a seamless approach to care in managing complex patients effectively.

Manage safeguarding issues as per policy.

Participate in task and finish groups to support service wide development.

Accurate and legible documentation in patients health records.

Work as part of a multidisciplinary team to provide specific interventions to improve outcomes for mothers, infants and families.

Utilise routine outcome measures to monitor effectiveness of interventions and adapt care appropriately.

1. To coordinate the care of service users carrying out assessment planning, implementation and evaluation of their care under the guidelines of the care programme approach and under the supervision of the line manager.2. To ensure that the service users care plan is developed to meet the assessed need of the individual and that unqualified staff are delivering care as laid down in the care plan3. To liaise with family members, carers and significant others when completing the assessment and developing a care plan4. To develop a risk management plan for service users, taking note of the contributions from other members of the team. Ensure that all significant people are aware of the risk management plan and are actioning it appropriately.5. To take responsibility for the safe administration of medication to service users ensuring that the Trust policies and procedures and the Nursing & Midwifery Councils regulations are adhered to.15. To ensure that all relevant information with regards to a service users care and treatment is documented accurately and legibly into the health record. To supervise and monitor the entries made by unqualified staff.

16. To under the guidance of a supervisor provide reports both in written and verbal formats as requested e.g. for a service user Mental Health Act Tribunal or Managers Hearing17. To contribute to teaching sessions in order to pass on knowledge and skills.18. To use skills gained through training and experience to de-escalate situations where service users become physically or verbally aggressive19. To ensure that unqualified staff members get appropriate support following violent or other untoward incidents20. To actively participate in service user reviews ensuring that service user needs are met and that the team has all the relevant information on which to base their clinical decisions.22.
